Western Sydney Local Health District Terms and Conditions
- I understand that my borrower account and barcode is my responsibility. I will advise the library of any loss or changes to details.
- I agree to take full responsibility for any items borrowed and will return items and pay any fee incurred for lost, damaged or overdue items. Unpaid fees may be deducted from my termination pay or result in the withholding of my exam results
- I am responsible for returning items by the due date and checking items have been renewed. Notices are a courtesy only.
- I agree to the terms and conditions for any items borrowed from the Nepean Blue Mountains Local Health District or The Sydney Children’s Hospitals Network.
- I authorise library staff at Nepean Blue Mountains Local Health District, Western Sydney Local Health District and The Sydney Children’s Hospitals Network to use the details provided to contact me regarding library matters and that the information collected is so that I can become a member of the library and utilise library resources and services.
- I understand that the information provided will be stored on eHealth servers and Prosentient servers (Koha library catalogue.)
- I understand that all documents requested by me electronically are subject to copyright restrictions, as per sections 49(1) of the Copyright Act 1968.
- If I am an employee, library staff may use my details to create a MyAthens registration for access to online resources. I understand that the information provided will be stored using My Athens servers.
- Auburn and Westmead only - When working in isolation after hours I understand it is recommended I inform security that I will be in the Library.
